GDCP: Why, When and How! – Session 1

This is a 1.5 day workshop.

SESSION 1 is 9.30 am to 4.30 pm, 15 August.

SESSION 2 is 9.30 am to 12.30 pm, 12 September.

Both sessions are at the same venue.

By booking into SESSION 1 you're also booking into SESSION 2 and should attend both sessions.

SESSION 1

This full-day workshop gives participants the information and practical strategies to support effective goal setting and person-centred care planning.

Participants have the opportunity to discuss current challenges and work collaboratively to generate effective and sustainable solutions.

The workshop covers:

✅ Best practice approaches to goal setting with clients and carers

✅ Strategies to understand client’s needs and priorities and how to translate these into tangible goals

✅ How to effectively engage clients and carers in GDCP (including clients who have dementia, communication difficulties or limited capacity for effective planning and decision making)

✅ How to develop and document person-centred care plans

✅ An introduction to the organisational systems that support effective GDCP.

Lunch and refreshments are included.

SESSION 2

This is a 3 hour follow-up that gives participants the opportunity to review progress, discuss challenges and seek support and guidance about how to embed GDCP into their practice.

The session includes a collaborative review of organisational tools and systems to support effective practice.

Who are the sessions for?

The workshops are designed for staff who are responsible for assessment, goal setting and care planning. Team leaders, coordinators and managers who directly oversee staff responsible for GDCP are also encouraged to attend.

No prerequisites required.
