Developing Effective Goal Directed Care Plans

This one-day in-person workshop provides you with tools & strategies for effective person-centred goal setting & care planning.

It gives you the opportunity to practice a range of techniques to engage clients in the goal setting process and work through case studies to put these skills into practice.

The workshop covers

✔️ Effectively engaging clients & carers in Goal Directed Care Planning (GDCP)

✔️ Strategies to understand client’s needs & priorities + how to translate these into tangible goals

✔️ How to develop & document person-centred care plans

✔️ Reviewing care plans efficiently & effectively

✔️ An introduction to organisational systems, tools & templates that support effective GDCP.

The workshop is tailored to staff working in CHSP-funded services & aligns with the requirements of the Aged Care Quality Standards & CHSP guidelines. Case studies & examples that reflect a range of CHSP services will be used.

Who’s it for

CHSP-funded aged care staff who are actively involved in completing assessments and/or developing goal directed care plans. Team leaders, coordinators & managers who directly oversee staff responsible for goal directed care planning. No prerequisite training required.

Cost

No cost for CHSP-funded aged care providers.
