Advance Care Planning involves thinking about, talking about and perhaps documenting future medical preferences and values.
The workshop will offer valuable insights on supporting older people through the Advance Care Planning process and making sure values and preferences are respected, understood and implemented.

People working in CHSP-funded aged care who may have conversations with older people about Advance Care Planning. This includes support and intake workers, coordinators, case managers and allied health professionals.
