Advance Care Planning

This online workshop is a great opportunity for people working in CHSP-funded aged care to enhance their understanding of Advance Care Planning, its importance, and how it can be integrated into daily conversations. 

Advance Care Planning involves thinking about, talking about and perhaps documenting future medical preferences and values.

The workshop will offer valuable insights on supporting older people through the Advance Care Planning process and making sure values and preferences are respected, understood and implemented.  

What You’ll Learn:

  • Advance Care Planning in 3 steps – a simple, practical approach

  • How to appoint a Medical Treatment Decision Maker

  • Tips for talking to older people about their medical preferences and values

  • Understanding Advance Care Directives, including values and instructions.

Who's it for?

People working in CHSP-funded aged care who may have conversations with older people about Advance Care Planning. This includes support and intake workers, coordinators, case managers and allied health professionals.

Cost

No cost for people working in CHSP-funded aged care.
